Are licensed nurses on duty around-the-clock?
Absolutely. As a skilled nursing facility, we provide 24 hour, 7 days per week licensed nursing staff in the building and on duty.

What is included?
Room and Board, meals, nursing care, social services, activities, housekeeping and laundry.

What is not included?
While not meant to be all-inclusive as a list of non-included items and services, the following is meant to give an idea of what is the resident's responsibility. Ancillary services such as diagnostic procedures, pharmacy, physician services, rehabilitative therapies, etc. are not included in the Room & Board rate. Supplemental services like beauty shop and personal item shopping are also the responsibility of the resident.

How often will my loved one, be seen by a physician?
At least monthly for the first 90 days and then every 60 days. A physician can visit more often if the patient is ill or needs to see the doctor.

How do you match roommates?
Personal consideration is given to roommates' preferences and prior history. Like personalities are paired together whenever possible.

Do you have private rooms?
Yes, private rooms are available. If a private room is not available at the time of request, you would be placed on a waiting list for the next available Private Room.

Do residents have personal items in their rooms?
Yes, in fact it is strongly encouraged in the hope of creating a familiar space for each resident to call home.

How does Clear Creek Care Center prevent abuse?
Extensive abuse prevention efforts have been employed at Clear Creek Care Center. Our staff is educated on a regular basis on how to recognize abusive patterns and how to report abuse allegations. Residents are also trained how to recognize and report abuse.

Is a grievance procedure available to residents?
Absolutely. The grievance procedure is integral to the Resident Bill of Rights.

What security measures are in place?
All visitors are required to sign in at the front desk during business hours. After business hours, the facility doors are locked and access to the facility is available only with the charge nurse's knowledge.

Are state Health Department inspection reports available upon request?
Yes. In fact, the reports are available to all visitors and residents without having to request them. The reports are available in the lobby for general public review.

HELP WITH CHOOSING A CARE CENTER

How do I start looking?
Ask your doctor or other health care professional for referrals. Talk to friends who also are searching, or who have a friend or relative already in nursing home care.

What questions need answers to make the best choice?
One should always review the current health department survey report. Also, check with the local Ombudsman for their experience with the facility.

Are special needs required?
Special care needs of the resident should always be considered: Whether the resident will require a ventilator, psychiatric care or extra supervision due to wandering or combative behavior. Although the majority of nursing homes are licensed as "skilled nursing facilities," they generally provide both skilled and "custodial" care.

What is the method of payment?
There are several methods. We accept private pay and private insurance payment as well as payment from Medicaid, Medicare and Managed Care plans.

How important is location?
It is important to choose a nursing home that is close and convenient to the person(s) who will be visiting the resident most often. When family members and friends are close enough to visit frequently, they can monitor the resident's condition, participate in care planning and respond quickly to emergencies.
