Pre-Admission

An assessment of types will be completed prior to admission by the Director of Nursing and Resident Services in coordination with the facility Administrator in order to find the best possible placement for the resident. The Director of Nursing and Resident Services will be reviewing previous and current medical diagnosis, along with current medications. The resident needs to have a current history and physical at the time of assessment. If the resident's physician will not follow them at Wheatridge Manor, the facility will assist the resident in finding a physician.

Admission Checklist

Prior to admission, to help facilitate a smooth admittance for you or your loved one to the facility, a number of forms must be completed. Please plan to allocate the necessary time with Resident Services and Bookkeeper, as there are a number of parts to the process. Printed below, is a partial list of information and items that you will need to accompany you to assist in expediting the admission process. These forms are required PRIOR to admission.

  1. Insurance information and cards such as Medicare, Medicaid, or and *HMO such as Secure Horizons, AARP, and Colorado Access
  2. Birth date and birth place of resident
  3. Physician orders (if coming from home)
  4. History and Physical from current physician
  5. Advanced Directives such as:
       Living Will
       Durable Power of Attorney
       Do Not Resuscitate Form
  6. Social Security Card
  7. Pharmacy preference with telephone number
  8. Responsible party, emergency address, and telephone numbers
  9. Guardianship and/or Conservatorship papers

Personal Items

Wheatridge Manor suggests that the residents bring a seven to ten day supply of comfortable, machine washable clothing. All clothing items should be marked with a permanent marking laundry pen so they can be laundered and returned to the resident. If you need a marking pen or assistance with marking the residents clothing, please notify a staff member prior to admission. Family members can do laundry if they wish, but we still ask that all items are marked just in case they are laundered by the facility. A suggested clothing list is as follows: pants, shirts/tops, dresses, sweat suits, socks/stockings, underwear, pajamas, bathrobe, shores with non-skid soles, slippers, sweaters.

Valuables

Residents have the option to keep personal funds in a resident account at our facility. We advise a resident to keep no more than a few dollars in their rooms. Valuables, such as jewelry and heirlooms, are best left at home. Most jewelry items are not recommended, especially those of value, whether monetary or sentimental. Such items are easily misplaced by residents. Please make sure that all valuables are registered on the personal possession sheet in the resident's chart upon admission and marked with the resident's name at the time they are brought into the facility.
